In the 1960s a 20-year-old girl nicknamed "Babydoll" (Emily Browning) is institutionalized by her widowed stepfather at the Lennox House for the Mentally Insane after she is blamed for the death of her younger sister. The stepfather bribes orderly Blue Jones to forge the signature of the asylum's psychiatrist to have Babydoll lobotomized so she cannot tell the authorities the truth about her sister's death.

In order to cope with her new stressful situation, she enters the hyper-real world of her imagination, and the lines between reality and dream begin to blur. She plans to escape from that imaginary world but to do that she needs to steal five objects before she is captured by an unknown adversary.

Her weapons include a nickel plated colt M1911A1 with small jewelry attachments, a Heckler & Koch MP7A1 (accessories include a silencer, tiny red dot sight and flashlight) and she utilises a katana for melee combat which she carries on her back.
